Exchange License Count

I know when the MSA logs into a mailbox, that's when it uses a license, and the used license count is shown in Help|About. Is that count real time, or does it update every hour like when we replace the license file?

I had a case where the count down of Exchange licenses did never start. If the MSA logs into a source or target mailbox, it uses a license?
